On June 18, 2014, at 5.00 p.m., Ministry of Health conducts randomization procedure, random selection of rayons and hospitals on a special E-program, in the framework of a project “Health Results-Based Financing” (RBF). Twenty-one selected due to randomization Hospitals will receive additional funding, about 9 million US dollars - for repairs, purchase of equipment, medical devices, etc.
"RBF" is a new project of the Ministry of Health of the Kyrgyz Republic, it started in the Kyrgyz Republic in 2013, and carried out with support of World Bank. The project aims at improving quality of care. It supports the government's objectives to increase efficiency and effectiveness of public expenditures of health sector through provision of sound and transparent framework for the financing of health care organizations based on the results. Its main objective is to address two important challenges in the health sector: improving quality of care for mother and child and more efficient use of funds. The main benefiters of the project are mothers and neonates, through improvement of quality of medical services. In addition, project implementers hope that infrastructure will be improved and material and technical provision.
According to Director of RBF project under Ministry of Health of KR Klara Oskombaeva, the selection procedure of hospitals will be held transparently and open with the participation of representatives of health organizations of Chui region and National Center for Maternal and Child Health, Jogorku Kenesh, Government of the Kyrgyz Republic, Ministry of Health and Public Supervisory Board, MHIF and non-governmental organizations and mass media. Program used in randomization excludes impact of man. It is based only on statistical data (on the amount of treated cases, beds, staff, etc). Totally in randomization 63 hospitals will participate, which are to be divided into three equivalent groups, 21 hospitals per each group. The first - with the use of intensive monitoring scheme and use of Scorecard with financial payments based on the results. The second - using intensive monitoring and application of Scorecard without funding based on the results. Third – will be control group. In these groups, all regions should be comprised, as well as rayon hospitals and general practitioners centers.
In the system of financing of hospitals RBF project will be based on the results and implemented performance, ie only if the quality indicators of quality of medical care present. For the evaluation of hospitals Scorecard was developed with a specific set of criteria and indicators.
Funding of hospitals on the basis of the results will be carried out by Treasury system. In order to prevent corruption in the evaluation of the hospital every six months counter-verification will be conducted by a group of external experts, including representatives of NGOs, professional medical associations, international partners working in the Kyrgyz Republic.
Monitoring of the project will be carried out by National Steering Committee, composed of representatives of the Government, the Ministry of Health, Ministry of Finance of the KR and MHIF.
Randomization procedure will be implemented by Jed Freedman, chief economist of team on development of researches of World Bank.
The total amount of grant funds for RBF project is USD 11 million. The project will last for 3 years and will last until the end of 2017.
Model of RBF project is currently being processed on the basis pre-pilot, Chui regional hospital.
