The Ministry of Health has conducted the third review of hospitals’ performance at the end of the first quarter of the last year, working with the support of World Bank on the project “Health Results-Based Financing” (RBF).
As it is known, in June, 2014, in the selection of hospitals 63 rayon and rural hospitals have participated, including General Practioners Centers (GPC). By random method they were divided into three groups, in first of which it was decided to carry out a quarterly assessment of medical services’ quality with payment of project funds, in the second to conduct only quarterly assessment of the quality of medical services without payment of funds and the third group will be a control group. There were 21 hospitals and GPCs in each group.
According to opinion of Mrs. Memeryan Shimarova, expert on monitoring and evaluation of RBF project, in total 4 reviews were conducted on performance of health care organizations, the first was baseline review. They are conducted at the end of the quarter. In the last third review, from first group of hospitals - Ak-Syiskaya territorial hospital (TH) became leader and scored 923 points and Balykchy city TH scored 900 points. The lowest points -342, as in the previous review was General Practioners Center (GPCs) Jany-Jer in Batken region. In the second group Uzgen TH was the leader, scored 774 points, Nookat TH - 772 points. Among the laggards in the second group the lowest score (143.5) belong to GPC named after Malabasheva, village Ozgorush in Toktogul region. GPC Orlovka was also included in the list, which scored even fewer points - 142.2. Compared to previous review this hospital, unfortunately, decreased its performance.
In general, in the results of all four surveys, indicators of hospitals’ performance improve, and according to baseline review if the results were less than 10%, then today the first group showed 60% and 47% - in the second. As shown in the third review, the gap between first and second groups increased.
The assessment shows that growth rates in both groups observed in all four surveys. The gap between the first and second groups began to increase. If in the second review the difference is equal to 81 points, then in the third -133 points. As it is known, the first group hospitals receive incentive payments from RBF project.
"If you look categories of Scorecard scores on patient satisfaction, it is increased significantly. This indicator is one of the results of the project” - said Memeryan Shimarova. Better care is provided to neonatal, hospitals are undergoing major structural changes in creation of conditions for patients, medical supplies, drugs and blood components. Hygiene and management in hospitals were improved significantly. Scorecard consists of seven categories. First is management, hygiene of medical treatments. 2. On-site training, capacity building/ library. 3.Patient satisfaction (interviewing of 10 people). 4. Quality Assurance (survey of internal system of controlling quality of hospitals). 5. Surgery (survey of quality of medical care in surgery). 6.Maternity Department (survey of quality of maternal care). 7. Pediatric/neonatal department (survey of quality of medical care to neonatal and children up to 5 years of age).
The more points earned by the hospital on these categories, the more additional funds its budget receives. "
