Project Secretariat of the Ministry of Health "Health results based financing” project (RBF) conducts review of the activities of the hospitals on the results of the first quarter of 2015. RBF project is implemented in Kyrgyzstan since 2014 with the support of World Bank. At the end of April, this year, the subject of attention of RBF Secretariat was Talas region’s hospitals and, in particular, Talas Territorial Hospital named after Karakoichu Toktogulova. It is a part of the first group of regional hospitals participating in RBF project, which for its performance earned scores, received additional funding. During its working visit to Talas region RBF Project Secretariat has met with Deputy Plenipotentiary Representative of State regional administration Ryskeldy Botikov, achievements were discussed, needs and requirements of healthcare organizations. Mr. Massashvili Nadar, an expert of World Bank on financing also participated in that meeting.
Klara Oskombaeva, Director of RBF project: The project is aimed primarily at improving the delivery of health services to mothers and children. It does not seek specific goal to reduce maternal and neonatal mortality rates, but interested and achieves results in the improvement of conditions of hospitals at the regional level. If the hospital improves its conditions, makes more perfect management system, and accordingly catches up indicators. Today, there is improvement in the hospitals’ performance of Talas region. Almost a year ago, from 1000 scores, they obtained maximum 100 points. Today, these results significantly increased. For half a year they made very good progress. Only Manas territorial hospital (TB) earned the lowest score - 439. Talas TB scored 562 points, Kara Burinskaya TB - 582 and Bakai-Ata TB - 589 points. There have always been problems with heating and water. Today in each ward there is water, heating. To prevent intrahospital infections, repairs were carried out in sterilization departments, rooms of utilization of waste. Each hospital and offices created a Quality committee. Patient satisfaction increases, kickbacks minimized. We try to bring to every doctor who is in charge of quality, knowledge and skills, how to work for the result.
Sveta Omuralieva, Director of Talas TB: Talas territorial hospital was designed for 60 beds, consists of five departments: maternity, surgical, therapeutic, infectious and outpatient diagnostics. She works in RBF project from June 2014. This is a very good incentive for staff. In the first review, the team scored just 68 points, in the second - 285, third - 474, and in the last review has earned 562 points. The more points we gain from RBF project, the more money we earn. According to the latest results of our activity we have earned in addition to the budget 375 thousand soms. The annual budget of the hospital is very small, only 15 million soms. 80-85% of all funds are mainly spent on salaries. Local authorities also have enough possibilities to help us. And we have a furnace heating and 33 Contremarques. We heat with coal on the cost of our own budget. We have already discussed with Quality Committee of what we’ll buy for the earned additional money. We need to buy high-frequency amplifier UHF, laboratory centrifuge and drying box. The hospital was built in 1938, it is not surprising that it is gradually destroying. It would be, of course reasonable to build a new building, but we can not spend money resources for construction. We have allocated 29 thousand to repair the maternity ward. 20% of the RBF funds will go to the salary raise. Here we employ 120 people. Except salary increase, we have more opportunities, purchase of drugs, medical equipment. From RBF funds, we bought sinks, equipment, wardrobes for medicines, couches, gynecological chair, first aid kits for each department, hygiene in the amount of 170 thousand soms. From the last funds, we have directed 26% of the money to solvency one of the most complex problem - water supply and repair of the building. Well, it is good that the assessment is conducted by peers from other hospitals. The first and second reviews of evaluation of our activities have been carried out by experts of RBF and observers from General Practicioners’ Center of Suusamyr Territorial Hospital of Moscow rayon of Chui region. The third time, the review was carried out with participation of peers from Jets-Oguz GPCs. Additional funding from RBF project helps us very much. No other organization would give them. A big plus is that the quality of work improves. We analyze our weaknesses and try to correct them. Medical workers have stimulus. They want to get high scores and earn more. If there are more projects such as RBF, our health care will improve better.
Attokur Shadiev, Coordinator of Health Care in Talas region: Since independence, the hospital, FAP and FGP (family group practicioners) in Talas region almost were not repaired. The RBF project, has brought positive improvements, and as a result, we have got good medicines, equipment and funds for repair of hospitals. We have opportunity to repair maternity wards. From four regional hospitals only one hospital was included in RBF project for additional financing, it is Talas Territorial Hospital. Others work in the second group, and also showed good results. We hope that together with the RBF, we will continue to improve.
Narynbek Toktosunov, Head of Department of Social Development and monitoring of Talas regional state administration: Talas territorial hospital gets helps from company "Jeruy" and local budget. It's good that it participates in RBF project. Opinions of physicians and nurses is changing. They realized that with the improvement of medical services they increase interest. It is noteworthy to say that in every department, boxes for comments and suggestions were installed, which are discussed each month on Quality Committee. Feedback influences very positively on the factors to eradicate corrupt practices. If earlier in conversations with patients, they complained, "All physicians take money, they need to put it in their pocket!", today they say that the conditions are good for treatment, they does not have to pay for normal treatment, the hospital provides all necessary medicines with which the recovery go faster. My opinion is that that due to RBF payments, not only conditions will improve for patients and health care workers, but corruption will obligatorily reduce.
Satkyn Raimbekova, Head of Department on therapy of Talas TH: Recently, we have made evaluation of Kochkor hospital, which shows good results, and then I have an excitement and stimulus. I want to update everything. We will try to earn more. Since the beginning of RBF project activities, we have created Quality Committee. There is one in each hospital, and in each department. And there is additional salary payment. Now we are thinking about to hire a specialist to have trainings on earned funds on theme "Pocket Guidelines". We really need this training.
Sabira Alymkulova, Head nurse of therapy department of Talas TH: RBF helped us with purchase of wardrobes. In each ward, we now have washbasins. We really need computers, Internet. And we dream that we’ll have a shower and need to buy a couch.
Saltanat Aldakeeva, Nurse of Central sterilization department of Talas TB: I graduated from Talas Medical College. I have been working in the hospital for five years. Now working conditions improved markedly. I get five and half thousand KGsoms per month. I hope that with RBF project my salary will be higher.
